ALT Legacy is a consulting practice working at the intersection of healthcare, transportation, mobility, and Medicaid systems. We partner with NEMT providers, healthcare organizations, and mission-driven operators to fix the operational gaps that quietly block access for the people who need it most.

Our Story

ALT Legacy was founded after witnessing a recurring failure inside Medicaid transportation: people missing critical medical appointments not because rides were unavailable, but because of compliance gaps, credentialing breakdowns, and operational inefficiencies that nobody had fixed.

The problem was never a lack of services. It was a lack of infrastructure.

That observation became the foundation of ALT Legacy. What started as a focus on NEMT provider readiness has grown into something broader, consulting that addresses the full ecosystem of access, from transportation and mobility aids to healthcare navigation and systems-level equity strategy.

The through line has never changed: when operations work correctly, communities experience reliable, equitable outcomes. When they do not, real people pay the price.

About the Founder

I am Keayona Thomas, founder of ALT Legacy.

Before launching ALT Legacy, I held senior roles at two organizations that sit at the center of the Medicaid ecosystem; a nonprofit NEMT brokerage and a Medicaid managed care organization serving complex health plan populations across Massachusetts, Connecticut, New Hampshire, Rhode Island, and Pennsylvania.

That combination is rare. Most consultants in this space have seen one side. I have worked inside both reviewing what brokers require, what health plans flag, and what providers face on the ground every day. The populations I served had no margin for system failures. Their ability to access care depended on operations working correctly.

I did not start ALT Legacy to offer general consulting advice. I started it because I watched the same operational breakdowns repeat across providers and organizations, and I knew exactly what was causing them and how to fix them.

ALT Legacy is also a personal mission. The work is bigger than consulting. It is about building something that lasts, for communities that have been underserved long enough.

What We Believe

Sustainable systems create sustainable access. Providers and organizations can only perform as well as the infrastructure behind them. When systems are compliant, documented, and operationally sound, the people who depend on those systems get what they need.

ALT Legacy drives immediate operational impact while funding long-term community benefit. Through ALT Legacy Care Initiative (ALCI), we address structural barriers for underserved populations — starting with low-income older adults in affordable senior housing and building toward something much larger.
