Our Story
ALT Legacy was founded after witnessing a recurring failure in Medicaid transportation: people missing critical medical appointments not because rides weren’t available, but because of compliance gaps, credentialing breakdowns, and operational inefficiencies that nobody had fixed.
The problem wasn’t a lack of services. It was a lack of infrastructure.
That observation became the foundation of ALT Legacy. We started by helping transportation providers navigate Connecticut’s Medicaid NEMT credentialing and broker approval process, and we’ve built on that same systems-first approach ever since.
Today we work with NEMT providers across New England to build the compliance frameworks, workflows, and documentation systems that allow essential services to reach the people who need them most.
About the Founder
ALT Legacy was founded by Keayona Thomas, a consulting professional with direct experience on multiple sides of the Medicaid and managed care ecosystem.
Prior to launching ALT Legacy, Ms. Thomas held roles at both a nonprofit transportation brokerage and a nonprofit managed care organization serving complex health plan populations. That combination of experience, working inside a health plan and inside a transportation brokerage, provides a rare vantage point: understanding what payers require, what brokers expect, and what providers face on the ground.
Across both roles, the populations served were among the most vulnerable, individuals with significant medical, behavioral, and social needs who depended on coordinated systems to access basic care. Responsibilities included transportation access coordination, compliance navigation, and service delivery support within regulated nonprofit environments across multiple states including Massachusetts, Connecticut, New Hampshire, Rhode Island, and Pennsylvania.
That inside experience is the foundation of ALT Legacy. We don’t just understand the compliance requirements; we understand why they exist, where they break down, and how to build systems that hold.

Our Long-Term Vision
ALT Legacy drives immediate operational impact while funding long-term community benefit.
Through ALT Legacy Care Initiative (ALCI), a developing nonprofit initiative, we address structural barriers for underserved populations. Phase I focuses on low-income older adults in affordable senior housing, tackling social isolation and mobility access barriers.
